Please fill out the below document for each bill. Requests will be reviewed by The Lost Ball Foundation and payment (partial or full) will be made ASAP, pending board approval and available funds.
While The Lost Ball always aims to fulfill 100% of needs, we are not always able to meet those desires secondary to availability of resources.
